The placement process itself often includes the following:  

• Family members, guardians and/or medical professionals contact us on the behalf of the client seeking/needing AFH placement.
  • With their permission, through an initial screening telephone interview we obtain detailed formation about the client which is used in the AFH preliminary search process.
  • This information includes elder’s medical, emotional and social needs, monthly budget range for care services, and the desired geographic location of the new care home.
    • The obtained information will be kept private by the placement agent, and will only be used to exclude incompatible facilities.
• Before any further steps can be taken a nursing assessment must be completed which is required by the State of Washington for all AFH admissions.
• The placement agent and a qualified RN will need to meet with you and the client for a professional nursing assessment of medical, physical and social needs.
  • Anticipate granting access to appropriate medical records for this assessment and the asking of many questions about what has been and is now important to the daily life of the client.
• After all of the needs have been evaluated, we start with selecting 3 to 5 carefully screened care communities that best match the client's requirements and preferences, and schedule a visit at your convenience.
• Before any adult family home is recommended we also evaluate the existing clients in that facility so as to ensure compatibility between residents.
• We work for our clients until the search process results in the client finding the best adult family home that meets all their needs and expectations.
• AFH Options Placement Agent will personally show you and your loved one facilities that best meet individual requirements.
• As part of our excellence assurance program, we also follow up with you and your placed loved one to assure that everything is meeting your expectations. This is done at no cost to you or your loved one.
